The study focused on intravenous ketamine-assisted psychotherapy for individuals with moderate to severe alcohol use disorder. Using data from the Ketamine for reduction of Alcoholic Relapse (KARE) clinical trial, the researchers aimed to understand the role of ketamine's psychoactive effects in supporting abstinence. Dr. Will Lawn, the lead author and Senior Lecturer at King's, explained:
"Our previous work showed ketamine's promise in helping people with alcohol use disorder, but these new findings challenge the idea that its therapeutic benefits are solely due to psychedelic effects."
Beyond the High: Exploring Alternative Theories
The study revealed that participants receiving intravenous ketamine indeed experienced the expected subjective effects, such as altered reality, bodily dissociation, and distorted time perception. However, the research team suggests that these effects might not be the primary drivers of ketamine's therapeutic success. Instead, they propose alternative theories, such as ketamine's ability to alter brain networks related to addiction or stimulate the formation of new neural connections.
"Our findings open up new avenues for research," Dr. Lawn added. "We need to explore these hypotheses further to understand ketamine's mechanism of action fully."
The KARE Study: Unraveling the Mystery
The KARE clinical trial, supported by the Medical Research Council, involved 96 adult participants at two clinical research facilities in England. Over three weekly dosing sessions, participants receiving ketamine reported intense psychoactive experiences, including altered reality and out-of-body sensations. Importantly, these effects remained consistent, indicating little tolerance development.
However, the study found no significant evidence that these psychoactive experiences mediated ketamine's therapeutic benefit in reducing alcohol consumption. In other words, the intensity of the drug's subjective effects did not predict the success of treatment.
MORE-KARE: The Next Step
Professor Celia Morgan, the lead of the KARE study and now the principal investigator of the MORE-KARE clinical trial, emphasizes the need for further exploration. MORE-KARE aims to recruit individuals with alcohol problems from across the UK to delve deeper into the roles of brain connection, function changes, and optimal dosing.
"Alcohol use disorder is a significant public health concern, and we need to diversify treatment options," Professor Morgan stated. "Our research highlights the need to understand ketamine's therapeutic action fully."
